To apply for admission to Mary Health, you may download the application or request an application by calling the front office. All applications must be completed in entirety and received in the business office before the applicant is placed on the waiting list.

After receipt of the application, the applicant is immediately placed on the waiting list. The effective date of the application is the date that the application is received. Each applicant resident is assessed by the Interdisciplinary Team to ensure that Mary Health of the Sick can provide a safe and secure environment for the applicant. The waiting list is not always sequential by date of application. Admission to Mary Health is based on many criteria as well as the type of bed Mary Health has at the time, not just the date of application.

On the application form, there are two boxes to be checked which indicate whether the applicant is currently ready to be admitted or whether the applicant is not ready at the present time. Since there may be a period of time that an applicant has to wait before a bed that will suit her needs becomes available, many applicants apply before they are actually ready to be admitted. If the applicant is ready to be admitted at the time the application is submitted, the “ready now” box should be checked. If the applicant is not quite ready for admission, but wishes to be placed on the waiting list for future admission, the “not ready” box must be checked. If the “not ready” box is checked, the applicant or responsible party will not be called until he/she notifies Mary Health that the applicant is now ready to be admitted.

Each fall a letter is sent to every applicant requesting the applicant to call the business office to confirm her wish to remain on the waiting list. If there are any changes to the original application, this updated information should be mailed to Mary Health. All information is kept confidential.
